Licensed by the state of Oregon and certified by Medicare, St. Charles Home Health Services provides registered nurses, certified home health aides, physical and occupational therapists and licensed social workers who collaborate with you, your family and referring physicians to develop and provide excellent in-home health care treatment.

Physical and Occupational Therapists
  • Assessment for risk level and prevention of falls
  • Assistance with cognitive issues
  • Balance training
  • Family/caregiver training
  • Home safety evaluation
  • Maximizing mobility and functional independence
  • Recommendations for adaptive and assistive devices and equipment
  • Strengthening programs

Skilled Home Health Nurses 
  • Care plan development
  • Intravenous (IV) therapy
  • Medication management
  • Ongoing consultation with your physician
  • Pain management
  • Wound and ostomy care (WOCN)
  • Collection and delivery of lab specimens

Certified Home Health Aides
